It might a PS4 issue too. I remember my PS4 would randomly eject discs out while I'm playing. I fixed that by removing the tiny sticker on the bottom of the eject button. Your case seems more extreme though. Hopefully it gets resolved.

I hope so too, it's not game breaking but it is annoying having to reinsert my disc since I have no other choice XD My disc is clean too

Why you leave the game on with rest mode? Like, what is the reason for that?
Also @samuraihachi i had the same problem, i just cleaned up the thing and worked, but if it happens again, i'll isolate the eject button.

I play DOA almost every night for practice and once I'm done I just press the home button and put it on rest mode so I can pick up where I left off the next day.
