The costumes which don't load are only Kasumi as Yae (here like costume 39) and Phase 4 in her costume in Story Mode (here like costume 21). My game is the original.

Yep. The game doesn't crash. The costumes don't load but I can to choose it, BUT the fights with this costume also don't load. Understand???

An error in Swap mod. The Kasumi as Yae doesn't finish to load as in select screen as in fight. =/ Phase 4 Dark Costume too. =/

@mralex I loved the pack, but I miss vintage school uniform of Kasumi (that navy one) and Kasumi's recolored C2 on Phase 4. Will you released them later?
