You have to clear the game on hard in ng+ to unlock face paints and the hardest difficulty to unlock all of them. I didn’t know that before googling it just now & thought I could unlock them all on hard ng+ which is what my current playthrough is. >> Would have started on the hardest otherwise.- Chapstick
