DOA4 Arcade Stick: How awful is it?

Thicker than the previous one. Which means you can put another stick in without having to extend the base.

"Designed to replicate the authentic arcade cabinet." means it has both the straight (index rests on the second row) and curved button layout (index rest on the first row).

This is the same case as the SCV and TTT2 sticks. Also, it already comes with Sanwa parts so the only reason to replace is if you prefer Seimitsus or want to use clear buttons. The layout itself is based on the Namco Noir cab, with 2 extra buttons at the end.

Also, the PS3 version comes with 2 USB ports where you can stick a headset and keyboard in.
