Alpha-152 was very like killed from the hands of the fake Kasumi when the two fight; instead the DoA5 shape shifting boss is what Lisa think to be an example of Phase-4. In the end the boss and the laboratory containing apparently all the Phase-4 ready to the battle are destroyed, but the story reveal as Rig and Donovan don't think to what happened as a failure, just the opposite because ended to get the test that they needed for continuing their project.
Everything make think that the presumed Phase-4 boss and all the clones similar to it, in the end were just an hoax (or at least middle phase of the project and not the final one), seeing as a well working prototype of Phase-4 (sent in battle against Bayman for testing it), appear to be completely different from the shape shifting clone.
By the way your confusion very likely born because Team NInja decided to call everyone Phase-4 XD
In the end the clones are:
- Kasumi-Alpha
- Alpha-152 (that is Kasumi Alpha mutated).
- Fake Kasumi (very likely a clone of Kasumi of the same generation of Kasumi Alpha, only without the knowledge to be a clone)
- Shape shifting DoA5 boss (a clone very likely defined wrongly from Lisa part of the Phase-4 generation)
- Kasumi Alpha Phase-4 (the DLC character that actually is a prototype of the real Phase-4, sent on the battlefield just as final test)
- The real Phase-4 generation, or if called in another way, the final outcome from the prototype of Phase-4 (we still need to see them, could be clones similar to the prototype only without an human side, or maybe something completely different)
All this is crazy! XD (By the way the first four characters were killed and so are now out from the plot).
