I've seen it happen on The Throwdown as well as Forbidden Fortune. Perhaps they have to get smashed through at least two breakables or something? I don't know the exact requirement but close hits seem plausible as well.

It happens when you break blow them into crates, or some objects.

No clue. I've done a lot of environmental combos, but apparently I've only done 6 mass destruction thingies in my entire time playing this game.

I forgot to mention that these are specific objects that trigger mass destruction.
Here’s a self made list of what triggers mass destruction.
Unforgettable:
- Both Pool tables at Taylor’s bar area
- the benches in the great hall.
Road rage:
- the crates near the fallen truck (appears after you knocked them into the blue car that expands the stage)

Throw down:
- The crates (appear after you’ve knocked your opponent into breakable objects).
Note that after you’ve kicked them into a breakable object you can do a mass destruction twice. One you can do near the blue trash container and the other one is next to it.
The crates on both sides next to the cargo container trigger mass destruction as well.
